Ensuring that our high-quality outcome-driven behavioral, mental health and education services propel struggling children forward, away from the ill effects of poverty and racism, toward a future limited only by the scope of their dreams.JOB SUMMARYThe Intake Specialist/Family Team Conference Facilitator is responsible for the recruitment and processing of new referrals and the facilitation of conferences between the program, ACS, and the families receiving our services.PRINCIPLE DUTIES & RESPONSIBILITIESResponsible for managing the intake area of the program and ensuring program is in compliance with all metricsResponsible for representing the agency at ACS conferences to encourage families to participate in servicesResponsible for collaborating with the Program Director and Program Supervisors to dialogue around which Case Planner can accept which case.Responsible for ensuring Case Planners have all the needed information to conduct outreach to potential families.Responsible for following up with ACS Liaison/CPS staff to ensure that Joint Home Visits are conducted within 5 days and that cases achieve a disposition within 10 daysResponsible for facilitating family team conferences as needed and documenting the outcome in the programs databasesResponsible for managing the entire intake process from the initial connection with ACS/referral source, to obtaining the correct documentation, to outreaching with families, to processing the needed paperwork to open cases in our data base system and troubleshooting with the appropriate departments when delays arise.Responsible for monthly face-to-face outreach to market program to community schools, hospitals and other human service organizations.QUALIFICATIONS:MSW or a Master's Degree in another social service discipline (i.e. psychology, counseling, etc.)Must have at least 2 to 4 years of social service or other relevant experienceMust be skilled at engaging families and childrenMust be knowledgeable of Microsoft Office including Teams, Word, PowerPoint and Excel.Must be comfortable with collecting and aggregating dataMust be a quick learner and able to learn new systems quicklyMust have excellent communication and presentation skills
