Community Health Action of Staten Island (CHASI ) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities - people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Alternative to Incarceration (ATI) program provides intensive case management for those who are involved with the legal system and exhibit the need for substance use and/ or mental health treatment. The program facilitates access to care, treatment, and social services with the long-term goal of preventing future arrests and incarceration and managing their healthcare. The Intensive Case Manager plays a crucial role in supporting clients in the ATI program, focusing on those at a very high to high risk of recidivism. The program adopts a team approach where the client is an active participant, ensuring services are strength-based and trauma-informed. The Intensive Case Manager will maintain ongoing and enhanced follow-up with clients, ensuring they access and successfully connect to necessary services. They will assist clients in navigating their court mandates, providing support needed to meet legal requirements. By fulfilling these responsibilities, the Intensive Case Manager ensures that clients receive comprehensive, compassionate care tailored to their unique needs, helping them reduce the risk of recidivism and improve their overall well-being. DUTIES & RESPONSIBILITIES:
  • Conduct appropriate and effective screening and intake of eligible clients within two days of receipt of referral packet;
  • Develop and execute timely service plans with actionable and realistic objectives;
  • Procure the necessary consents, program agreements, and service plan signatures;
  • Facilitate program enrollment activities with the legal team including district attorneys, defense attorneys, and judges;
  • Provide all necessary referrals, initial appointment scheduling, accompaniments, and confirm linkages;
  • Coordinates services designed to integrate substance use treatment, mental health, and other recovery supportive services.
  • Participate in weekly conferences with court personnel, RCDA, and defense attorney;
  • Conducts individual and group trauma-informed supportive support sessions;
  • Outreach to the person's support system/ family/ emergency contact to gather information and support in service planning;
  • Document client and program progress for stakeholders and complete in a timely manner;
  • Present client progress reports to the court;
  • Participate in Quality Assurance activities and Quality Improvement projects;

  • Master's degree in social work, mental health counseling and/or full CASAC required.
  • At least 2 years of experience providing counseling services to individuals experiencing substance abuse or mental health issues.
  • Experience utilizing trauma-informed care and harm reduction models.
  • Bilingual (English/Spanish) highly preferred.
  • Strong assessment skills, care planning, and computer literacy.
  • Ability to work independently and be a team player with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates others to perform at their best.
  • Strong verbal and written communication skills, including capability to listen and follow instructions; able to give, provide direction and follow up on tasks; high attention to detail and accuracy. Be highly organized and able to multitask.
