As an Investigative Agent, you your investigate Major Unusual Incidents (MUI) in accordance with applicable law and regulations, involving persons with developmental disabilities. You will also conduct investigations of a variety of issues and personnel from a diverse group of service providers in accordance with standards of Ohio Administrative Code, including joint investigations also conducted with outside entities. An Investigative Agent also identifies trends and patterns of Unusual Incidents (UIs) and Major Unusual Incidents (MUIs) and recommends review of individual service plan (ISP) for the purpose of improving and enhancing the quality and appropriateness of services. You would also ensures adequate follow up has been taken by the provider/team to address the causes and contributing factors of MUIs and UIs.

For this position a bachelor degree and two (2) years of experience with conducting interviews, gathering and analyzing data, conducting assessments, and writing reports is required. You will also need a valid driver license with acceptable driving records. This position will require evenings and weekends, and locations convenient for person(s)/family(ies) served.
