Our top-tier benefits and great retirement packages can help you build a rewarding career and lasting future with the State of Illinois.Essential Functions* Directs Resident Care Plan meetings with the interdisciplinary team, updating resident's plan of care and monitors staff and resident compliance with the plan of care as required by the Illinois Department of Public Health and the Federal Veterans Administration* Provides direct patient care and primary nursing care services, such as applying medical treatments, administering, and charting medications* Plans, assigns, and reviews the patient care activities assigned to staff involved in administering professional emergency and health nursing services and care* Schedules and sends invitations to residents and/or Power of Attorneys at least two weeks prior to the date of the Care Plan meeting* Participates in Quality Assurance monitoring and attends Quality Assurance meetings* Performs other duties as required or assigned which are reasonably within the scope of the above dutiesMinimum Qualifications* Requires (a) graduation from an approved nursing education program resulting in an associate OR diploma degree in nursing and two (2) years of progressively responsible professional nursing experience; OR (b) a bachelor's degree in nursing and one (1) year of professional nursing experience; OR (c) a master's degree in nursing* Requires licensure as a Registered Nurse in the State of IllinoisPreferred Qualifications* Two (2) years of experience displaying visual, auditory, physical, and comprehensive ability in instructing and demonstrating clinical nursing techniques and documenting information* Two (2) years of experience evaluating patient conditions and behavior and preparing accurate, detailed reports* Two (2) years of experience conducting assessments and documenting patient data* One (1) year of experience as a Care Plan Coordinator* Extensive knowledge and understanding of State and Federal policies, procedures, regulations guidelinesConditions of Employment* Current Minimum Data Sheet (MDS) Certification or attainment within six (6) months of employment* Requires ability pass a background check* Requires ability meet all agency vaccine/health-related policies and guidance* Requires ability pass a drug screen and pre-employment physical* Requires ability utilize office equipment, including personal computers* Requires ability acquire and maintain Cardiopulmonary Resuscitation (CPR) certificate* Requires ability attend seminars, conferences, and trainings to stay current on methods, tools, ideologies, or other industry related topics relevant to the job duties* Requires ability to travel in the performance of job duties, with overnight stays as appropriate* This position is considered sedentary work as defined by the U.S. Department of Labor (20 CFR 404.1567(a)). Sedentary work involves lifting no more than 50 pounds at a time and requires occasional lifting, carrying, walking, and standing unassisted* On-call and overtime are a condition of employment, and you may be required to work overtime including scheduled, unscheduled, or last-minute overtime, and may be required to work alternating weekends. This requires the ability to work evenings and weekends.
