Description
Under the supervision of the Director, the Employment Specialist - Long Term Support position is responsible for providing ongoing skills training/coaching, job support, and follow-up to individuals who have been placed in competitive employment and require support to maintain the employment. In addition, the role is expected to support the placement of other individuals in competitive employment via job development activities. The Employment Specialist is expected to understand and advance the mission of the agency, as well as practice ethical and confidentiality standards. The individual in this role must adhere to all agency and program standards including those applicable through CARF, Vocational Rehabilitation, Public School Systems, HIPAA, NC MH/DD/SAS, Division of Health Service Regulation, and Medicaid as well as any other applicable funding or referral sources.
Responsibilities
- Conducts program orientation for participants entering services, or a new program of services, in accordance with agency and CARF standards.
- Maintains case records in accordance with agency, funding source, and CARF standards.
- Reviews case files and progress of person supported and develops/adjusts individualized person centered plans accordingly; recommends admissions actions.
- Coordinates or participates in person centered planning meetings including persons supported, family members, or other relevant parties in order to provide feedback regarding services and insuring appropriate goals are included.
- Completes service notes in accordance with agency and funding source requirements.
- Ensures authorization requests and billing are completed in a timely and accurate manner.
- Conducts at least twice monthly visits to employed individuals and employers to assure successful job performance by providing job support and job coaching services.
- Provides instruction/counseling in job maintenance, work related skills/behaviors, and additional areas related to employment based on observation in the community and feedback from supervisors and the individual's needs.
- Determines and coordinates ancillary services for individuals as well as informs/promotes individual rights and advocacy; assists persons supported in developing self-advocacy skills.
- Performs formal/informal job analyses as relates to accommodations and training needs of persons supported.
- Learns specific job tasks and company policies in order to provide job coaching services.
- Ensures compliance with Agency's and Employer's policies and standards and provides liaison services to employers, family, referral sources, and other parties.
- Establishes positive employer relationships in job development.
- Maintains appropriate community involvement to facilitate job development activities.
Required Education / Experience
- High School Diploma required. Preference given to persons with Associate's or Bachelor's Degree in human services or related field.
- Two years' experience in rehabilitation/providing services to persons with disabilities required. Equivalent combination of education/experience will be eligible.
Minimum Qualifications
- Must have valid NC Driver's License and driving record adhering to Agency standards.
- Must either qualify as a QP under MH/DD/SAS guidelines or be managed by a Qualified Professional.
Physical Requirements
- Position is considered medium, requiring lifting of up to 50 pounds.
- 75% standing/walking and the ability to bend, stoop, push/pull, and fine and gross dexterity skills when training persons supported on-the-job.
- Local travel is required.
- Requires visual observation and the ability to communicate with a variety of individuals.
