We are looking for a Registered Nurse for our Licensed Home Care team.Department & Position Overview:Provides professional nursing care and services to agency's clients according to Physician's orders, nursing standards of practice and the agency's policy and procedures- In Patients Home (the 5 Boroughs). Knowledge in Licensed Home Care Service Agencies. Every Day You Will:Provides care and services according to scope and standards of Nursing Practice and adheres to the Code of Ethics for NursesRespects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency's compliance program and privacy policy Follows the agency's policy and procedure regarding infection control measures through proper handwashing, bag technique, use of Personal Protective Equipment, principles of aseptic technique, exposure control/reporting, care of equipment, safe needle technique and other protocols consistent with agency's infection control programAssesses and regularly reassesses the client's needs and his/her response to care, treatment and educationDevelops implements, evaluates, revises, and provides input to the client's plan of care to include the physician's plan of treatment, problem list, nursing care plan and the aide's plan of care. Provides care according to the plan of care Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible mannerActively coordinates the care of the client through communication and written documentation to the physician, case manager, and supervisor, other disciplines involved in the client's care, labs, equipment, or supplies, and community resourcesSupervises ancillary staff involved in the client's plan of careProvides education to clients and families through assessment of the learning needs, development of teaching objectives and plan, implementation of the teaching plan, evaluation and revision of the teaching plan as needed Interprets the philosophy and objectives of the agency to clients, families, and others Participates in the agency's Performance Improvement ProgramPerforms other job-related duties as may be required or directed by the supervisor Specialized Skills and Competencies: Demonstrates knowledge and clinical expertise within the scope of practice of a Registered Professional Nurse practicing in New York State Excellent communication skillsExcellent organizational abilityMust be computer literate, including the ability to learn and use program-specific softwareAbility to direct home care workersDemonstrates knowledge of the New York State Dept of Health regulations for Licensed Home Care Service Agencies Physical Requirements:Will spend varied percentage of working time indoors or outdoors depending on the program and/or client needsWill encounter prolonged standing, walking, frequent lifting, positioning clients, pushing and/or transferring clients; lifting supplies and equipment; frequently requiring twists, stoops, bends, squats, and occasionally climbs stairsIdeal Candidate Will Possess:Bachelor's Degree in Nursing Active Registered Nurse license in New York State Must Have LHCSA experienceCurrent license and registration to practice as a Registered Nurse in New York State Must be computer literate, including the ability to learn and use program-specific software Minimum of 2 years clinical experience, 1 year of Home Care preferred Willing and able to work a flexible schedule which may include weekends and holidaysWilling and able to travel to Manhattan, Queens, Brooklyn, and the BronxWhy work at FreedomCare?We are here for you and committed to providing a best-in-class employee experience. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $100.00 and $130.00 per assessment at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.Salary Range (Per Assessment)$100-$130 USD
