Licensed Clinical Social Worker - Cordial Health : Job Details

Cordial Health is seeking a dedicated and compassionate Social Worker to join our team. We are a leading healthcare organization providing comprehensive mental health services to patients across Yonkers, NY. This is an excellent opportunity for a skilled LCSW who is passionate and committed to improving the mental health and well-being of individuals.

Responsibilities:

• Conducting comprehensive assessments to identify mental health issues, strengths, and treatment needs.

• Developing and implementing personalized treatment plans in collaboration with clients.

• Providing evidence-based psychotherapy and counseling to address a wide range of mental health concerns, including but not limited to depression, anxiety, trauma, and substance use disorders.

• Advocating for clients' rights and assisting them in accessing community resources and support systems.

• Collaborating with a multidisciplinary team of professionals to ensure holistic care for clients.

• Maintaining accurate and timely documentation of client interactions, treatment plans, and progress notes in compliance with legal and ethical standards.

• Participating in regular supervision, case conferences, and continuing education to enhance professional development.

Qualifications:

• Masters degree in Social Work (MSW) from an accredited program.

• Current LCSW license in New York.

• Experience providing direct clinical services in a mental health setting preferred.

• Strong assessment, diagnostic, and treatment planning skills.

• Excellent interpersonal and communication skills.

• Ability to work effectively both independently and as part of a team.

• Commitment to upholding ethical standards and maintaining client confidentiality.
