It effectively addresses patients' physical and mental health needs from home. By extending recovery services to eligible patients that hospital outpatient-based programs do not have the capacity for, allows Health Systems to serve a broader patient base and gain a new profit center. Further, our clinical model has the flexibility to scale across all heart and vascular service lines and beyond, reducing mortality risk for the chronic disease patients who need this care themost!About the RoleWe are seeking an experienced, Licensed Clinical Social Worker who is passionate aboutimproving the lives of patients and advancing equitable healthcare access. In this role, theLCSW will collaborate with a diverse team to conduct assessments, formulate diagnoses,develop treatment plans, create personalized care plans, and coordinate care within our team andalongside clinical teams from our partner Health System.If you are passionate about changing lives on a large scale, we look forward to hearing from you!What you'll do:Provide weekly virtual psychotherapy (45-50 min per session) to a panel of approx. 25 - 30 patientsEvaluate patient symptoms and risk factors to determine diagnoses and developindividualized treatment plansPerform initial patient diagnostic evaluation for mental health (or behavioral health)conditions, including screenings and assessmentsDevelop treatment plans that are supported by evidence-based therapies, currentliterature and standard practicesEnsure that treatment plans are supported by standard practices and/or current literatureFollow the plan of care to accomplish functions within established policies, procedures, and scope of practiceRe-evaluate treatment plans. Review and update treatment plans, as needed, to ensuresafety and maximum patient benefitPromote patient engagement and successfully achieve patient and program goalsCounsel patients using behavioral change techniques (ex. CBT, mindfulness,motivational interviewing) Communicate effectively across our integrated clinical team and our partner healthsystems to ensure the overall health and well-being of the patientProvide complete, accurate, and timely documentation of patient sessionsWhat you'll bring to the table:Skills/TraitsAn innovative mindset, and willingness to challenge the status quo of traditionalcardiac rehab experienceExperience working within integrated clinical teams focusing on polychronicand/or medicare populations is a plus. A passion for 1:1 therapy and driving patient behavior changeUnderstand relevant patient educational content and techniques (e.g.,Meditation, breath work, etc.) used in psychotherapyExcellent relationship-building and communication skills with the ability to talk tomany different cultural groupsPositive energy, patience and trustworthinessEffective and empathetic communication skills across phone, video andasynchronous messagingAbility to work in an agile start-up environmentA willingness to travel at least once per quarter for in-person team meetingsEducation/Certifications/LicensesA Masters Degree in Social WorkA valid and current Illinois LCSW license (required) (and willingness to obtainlicensure in additional states)Minimum of 3-5 years post- graduate experience in a medical and/or behavioralhealth setting.Posted On: Monday, January 16, 2023
