What you'll be doing

  • Responsible for overseeing the clinical team, ensuring accurate documentation, and maintaining comprehensive record-keeping standards.
  • Collaborate with treatment team members (internal & external) regarding consumers' progress through interdisciplinary team meetings, record review, and informal discussions.
  • Actively participate in departmental meeting, organizational committees, program design, and implementation of peer services as assigned.
  • Facilitate the coordination of consumer intakes, discharges, and transfers to and from other facilities programs, agencies.
  • Provide face-to-face case management or therapy services to assigned consumers as outlined in their Individual Plan of Service, and collaborate with consumers, their support networks, and team members as needed for coaching or intervention in complex cases.
  • Responsible for completing Service Activity Logs (SALs), developing case management resources, clinical procedures, documentation standards, therapy models, and outcome measurements, and contributing to evidence-based practice initiatives.

Anticipated Work Schedule

Regular Hours: Monday-Friday, 8:00am-5:00pm

On Call Rotation:

  • One day per week every other week
  • One weekend every other month

On Call Compensation Rates:

  • $50 additional for on-call duty Monday to Friday
  • $100 additional for on-call duty on Saturdays and Sundays
  • $150 additional for on-call duty on holidays

Job Requirements

  • Education: Master's degree in Social Work, Counseling, or Psychology from an accredited university or institution of higher learning.
  • Licensure: Must hold a Limited Licensed Master Social Worker (LLMSW) or Limited Licensed Psychologist (LLP) credential. Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC) preferred.
  • Experience: Required – prior case management experience in mental health. Preferred – two (2) years of supervisory experience within a mental health or rehabilitation setting.
  • Transportation: Valid driver's license with a clean driving record, per Hope Network insurance guidelines.
