Job Description :

Currently licensed as a Licensed Practical Nurse by the New York State Education Department. A valid and clean NY State driver license. Minimum 1 year of experience in a treatment or rehabilitation setting preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

JOB FUNCTION:

Provide clinic and community based psychiatric, substance abuse, and medical services as a member of a multidisciplinary treatment team within the Wyandanch Certified Community Behavioral Health Clinic. Complete all required documentation within a 48 hour period and comply with program, SAMHSA and OMH guidelines and regulations.

ESSENTIAL FUNCTIONS: Provide initial and ongoing assessments in behavioral health, substance abuse, and physical health Implement Psychiatrist, NPP or PA medical orders. Order necessary bloodwork, urine screening, perform history and physical, EKG etc. as instructed by provider Work alongside the Registered Nurse and take advisement Provide direct care, counseling, and education to patients and families according to established collaborative protocols, evidence based practices and standards of care. Assist in crisis management Support other members of the team during absence to include but not limited to: Administration of injectable medication Treatment Plan Updates Perform health services such as psychotropic medication treatment, health monitoring, and smoke cessation visits Provide medication management, education and administration Work with clients to develop an understanding of their symptoms, diagnoses and medications Screen for medication side effects and monitor withdrawal symptoms Administer medication (IMs) as needed Follow medication policy and procedure Responsible to conduct all assigned/scheduled appointments/visits Update Outlook calendar to reflect scheduled appointments Conduct Health Assessments and other required assessments (tobacco assessments, C SSRS, etc.) in accordance with program guidelines Maintain health office to allow for services to be delivered in a safe clean environment. Meet with all clinic clients a minimum of quarterly for Health Monitoring and Smoke Cessation for positive smokers Ensure timely submission and accurate completion of all documentation to coincide with the appropriate comprehensiveness to justify the respected CPT code Ensure progress notes written within 48 hours for every session, phone call, collateral contact, psychiatric, physical health and substance abuse assessment.

Complete treatment plans for assigned caseload Obtain prior authorizations for clients and report and track outcomes appropriately. Share in the joint responsibility of collaborating during clinical and staff meetings to aid in the integrative process of creating a patient centered, strength based treatment plan.

Oversee the accurate completion of all documentation Progress Notes for every session, phone call, collateral contact to be completed within 48 hours of contact and collaboratively with the client when applicable. Maintain compliance with incident reporting guidelines Report all incidents in a timely manor Responsible to comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description. Responsible for proficiency in all agency based IT applications Complete documentation in Foothold Awards EHR Complete agency training via KMS Provide education to clients and staff regarding standards of medical care Provide linkages to appropriate medical care providers when applicable Actively participate in staff meeting and provide medical insights to support team Responsible to perform all other job duties as assigned.
