Licensed Practical Nursesat VNH are an integral part of the care team, where they assess a patient's health status and complete nursing interventions in collaboration with the interdisciplinary team members and the patient.

The LPN will:

* Provide nursing services to individuals and families in their residence under the supervision of a registered nurse.

* Perform prescribed individualized patient care to patients as delegated.

* Assist the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.

* Observe, document, and report to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

Minimum Qualifications:

* Must be a Licensed Practical Nurse in Vermont and New Hampshire.

* A graduate from an approved school of practical nursing.
