Job Summary: Monitor conditions of individuals in assigned worksites, detect changes in condition or responses to medication or therapy, and report symptoms or changes to a physician or registered nurse. Administer medications, implement and monitor feedings, and perform catheterization orders, trach care, and deep oral and pharyngeal suctioning. Assist individuals in collecting urine, stool, and other specimens, and prepare and maintain charts and records. Perform inventory of medical supplies and medications, and perform direct care duties as assigned. Minimum Qualifications: * A license and current registration to practice as a practical nurse in New York State, or * A limited permit to practice as a practical nurse in New York State, or * An application on file for a limited permit to practice as a practical nurse in New York State. Note: If appointed with a limited permit, a license must be obtained within one year of the date the permit was issued. All candidates must also possess a valid Driver's license.
