The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. As federally qualified health centers, we serve the individuals in our communities, providing enhanced services, expanded hours and reduced prescription pricing, while raising the level of care. JOB TITLE: Licensed Practical Nurse
REPORTS TO: Nurse Manager
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
RESPONSIBILITIES:
* Provides nursing care under the direction of the registered nurse or practitioners, i.e. responsible for the nursing assessment of all patients; assumes an active role in interviewing patients and case management
* Assists practitioners with on-and off-site patient care
* Performs nursing procedures as needed, i.e. injections
* Maintains quality nursing care, nursing notes and plans
* Performs lab work, i.e. blood drawing
* Assists with patient education and counseling
* Assists in training of volunteers and residents as needed
* Must be familiar with emergency procedures and protocols
* Must be familiar with clinical policies and programs
* Must be familiar with referral procedures and back-up sources
* Charts and records the care administration of medication and treatments
* Participates in on-site and off-site screening projects, special programs and health education workshops as needed
* Acts as the patients' advocate and liaison for preservation of patient's rights
* Assists with compilation of health statistics
* Other duties as assigned.
QUALIFICATIONS:
* Licensed Practical Nurse in the State of New York;
* Basic CPR (BCLS) Certification required;
* At least two years of work experience as a Licensed Practical Nurse preferred.
* Capacity for covering LPNs at all sites if needed
* Bilingual in English and Spanish/Creole.
SALARY: Commensurate with experience
