The CTI Program is an evidence-based phase-based, time limited program designed to assist patients in accessing needed services during critical transition periods.Reports to: CTI Program Director*QUALIFICATIONS:*● Licensed Practical Nurse (LPN) with current New York State license● At least two years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement or limited social support;● Extensive knowledge of community treatment, support services, and resources● Highly organized and self-motivated● Spanish/Creole Speaking preferred● Proficient in computers● Travel or Special Conditions/Requirements: *Traveling required*.● Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.● Ability to prioritize tasks and manage time effectively in a fast-paced environment.● Attention to detail and strong problem-solving skills.● Commitment to maintaining patient confidentiality and privacy.*JOB DESCRIPTION:*● LPN will carry a caseload, providing integrated CTI services to patients in transition from inpatient units and emergency departments.● The LPN will address medical needs through assessing basic health and medical needs within their scope of practice.● Coordinating health care with community medical providers (e.g., primary care physicians, specialists, etc.).● Provide rehabilitation, education, and support services within their scopeof practice.● Coordinates consumers' health care services, providing psychoeducation to consumers about health and medications, assessing consumers' mental statuses;● Coordinates with prescriber on consumers' medication management needs, health care services, maintaining consumers' medication administration schedules;● Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, and primary health care providers.● Coordinates to provide in person and telehealth walk in services as per discharge instructions.● Completes and signs progress notes and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health, Mental Hygiene (DOHMH) guidelines, Medicaid billing standards and regulations and IDCC policy and procedure;● Participates in team and clinical meetings to collaborate and discuss clinical issues and clients' needs with team members;● Uses motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services.● Other projects as assigned.*BENEFITS WE OFFER: *● Medical including Dental and Vision● Cafeteria Plans: FSA, Dependent child care plan, qualified transportation plan● Up to 3% 401k Profit Sharing plan automatic contribution for eligible employees● 401k Additional 4% match of your contribution from your salary● Employee Assistant Program Services (EAP)● Community Mental Health Loan Repayment Program for qualifying candidate● Paid time off (PTO)● Referral Bonus for qualifying positionsInterborough is an EEO employer and provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regards to race, color, religion, sex, national origin, disability status, genetics, protected veterans' status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state and/or local law and encourage women, veterans and other minorities to apply.Interborough Developmental and Consultation Center is committee to Diversity, Equity, Inclusion and Belonging (DEIB) in the workplace.
