Licensed Practical Nurse - Tandym Group : Job Details

A health services organization in New York City is currently seeking a new Licensed Practical Nurse (LPN) for a promising opportunity at one of their group homes in Queens. In this role, the Group Home LPN will be responsible for providing overall medical oversight for program individuals.Responsibilities:The Group Home LPN will

• Provide observation, monitoring and documentation for all Individuals served on presenting health needs
• Administer medications and handle the inventory of all medication
• Review the medical data sheets
• Perform medical rounds, nursing procedures and other treatments as required
• Maintain accurate, neat, concise, timely documentation in all medical and agency records
• Participate in Interdisciplinary and other health-related meetings as requested
• Assist the Registered Nurse in addressing health teaching needs of the Individuals we serve, their families, and staff
• Perform other duties, as needed

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Licensed Practical Nurse (LPN)
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
