A reputable community-based healthcare organization is currently seeking Licensed Practical Nurses for their skilled nursing facilities located in Brooklyn. The base pay range for this position is $36 to $42 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. Responsibilities:

• Provide direct patient care, including catheterization, tube feeding, and IV insertion.
• Conduct patient assessments and monitor vital signs to ensure optimal health.
• Maintain accurate electronic health records (EHR) and adhere to HIPAA regulations.
• Assist in critical care situations and triage patients as needed.
• Collaborate with healthcare teams to develop and implement care plans.
• Educate patients and families on health management and treatment options.

Qualifications:

• Licensed Practical Nurse in NYS
• BLS Certification
• At least 2 years of prior nursing home experience
