What You'll Do:
  • Provide Direct Patient Care: Deliver compassionate and personalized nursing care to patients in their homes, ensuring comfort and well-being.
  • Perform Assessments and Administer Medications: Conduct thorough patient assessments, administer medications, and assist with treatments as prescribed.
  • Collaborate with the Care Team: Work closely with RNs and other healthcare professionals to develop and implement individualized care plans.
  • Educate Patients and Families: Provide education on care plans, medication management, and health maintenance to patients and their families.
  • Accurate Documentation: Maintain precise records of patient care to comply with Medicare and company standards.
Competitive Compensation:
  • Hourly Pay Range: $28 - $38 per hour, based on experience and qualifications.
  • Experienced LPNs (1-2 Years Home Health Experience): $28 - $32 per hour
  • Advanced LPNs (3+ Years Home Health Experience or Certifications): $33 - $38 per hour
What You'll Need:
  • Active LPN License: Must be in good standing and valid in the state of Virginia.
  • Experience: At least one year of Medicare home health experience is required, demonstrating your ability to manage patient care effectively in a home setting.
  • Strong Organizational Skills: Ability to manage time efficiently, prioritize tasks, and maintain detailed documentation.
