- Monitor vital signs and assist with ADLs (e.g., bathing, dressing).
- Provide basic wound care and manage chronic conditions.
- Observe and report patient changes; document care provided.
- Collaborate with RNs, physicians, and the care team.
- Educate patients and families on care plans.
- Follow infection control protocols and use PPE properly.
- Experience: 2+ years in long-term care.
- Licenses: Active NY LPN license, BLS certification.
