The Opportunity

The Link-Age Program is a grant funded program by NYS Office for Mental Health in a triple partnership with Family & Children's Association, Central Nassau Guidance & Counseling Center, and NYS Office for Aging. The program is focused on providing short-term intensive case management, counseling, health and psychoeducation, and peer services. We provide linkage to long term care, community supports and services to seniors 55 years+, residing in Nassau County that are facing behavioral health concerns, substance use disorders, challenges related to aging, disconnection from services, and struggling to access/connect to medical care. focus on person centered care using a strengths-based approach.

As a Link-Age Intern, you'll learn how to provide clinical case management while managing your own case load. You'll learn about the available resources and service providers in Nassau County for the aging population and have opportunities to network with other organizations. You'll learn how to conduct mental health, substance use, and aging assessments/screenings, identify treatment goals, and assist individuals who are struggling to connect with services. There is opportunities to participate in professional development trainings and facilitate a mental health focused support group at local senior centers.

Responsibilities

+ Provide a blend of telehealth and in-home visitations to clients while collaborating with program staff to address client's needs.

+ Maintain a client case load, case records, assessments, and statistics as required by the standards set by Family & Children's Association and NYS Office for Mental Health grant guidelines.

+ Provide case management services to clients such as: assisting clients with completing forms/applications to apply for services & programs; provide information/referrals to clients as requested; address barriers to connecting with services/ resources.

+ Accompany clients to appointments to provide support and advocacy, when needed, for clients regarding entitlements and other basic needs; represent them at DSS, SSA, etc., when they are unable to represent themselves in-person or via telehealth.

+ Provide supportive short-term counseling via tele-health or in the client's home

+ Participate in supervision with Program Coordinator and weekly Team Meetings; participate in agency trainings and workshops.

Qualifications

Interest in working with Geriatric/Older Adult or SMI population preferred

Must be enrolled in a Master level program for Social Work or related profession (1st or 2nd year)

Valid New York State driver's license with a safe and responsible driving history

Schedule

+ Minimum in-office 2x week (21 hours/3x a week)- technology provided

+ Office is open Monday-Friday 9:00am-5:00pm
