Sun River Health is currently seeking a full-time Licensed Master Social Worker (LMSW) / CASAC based out of our busy Peekskill, NY health center.This position provides crisis intervention, biopsychosocial assessments, individual, family and group counseling and/or case management services to HIV positive patients, their partners, caregivers and family members, as well as other patients with issues related to chronic illness, aging, maternity, domestic violence or other social problems. This position also provides education and counseling to patients and their significant others in need of addiction treatment counseling services. Performs job duties in compliance with systems and job specific standards.*ESSENTIAL FUNCTIONS** Performs a mental status exam on all new Mental Health patients and reviews with psychiatrist/primary care provider.* Provides a comprehensive mental health intake/assessment for all new patients referred by primary care provider or other service provider for medical and/or social/emotional issues.* Maintains data and case records as required and prepares necessary reports.* Provides individual, group and family counseling to patients and their families.* Provides creative prevention services to individuals at risk of infection and other chronic medical conditions or negative health or social factors.* Provides case management services as needed to support primary care.* Responsible for the accurate and timely completion of clinical documentation in patient records.* Provides crisis intervention/emergency evaluations to patients when required.Required Education & Skills:- Masters Degree in Social Work (MSW)- Bilingual English and Spanish- Active NYS LMSW licensure- Active OASAS Program CASAC (Credentialed Alcoholism & Substance Abuse Counsel)Required experience:- 2+ years of experience working with patients in a healthcare settingSun River Health is a network of over 40 Federally Qualified Health Centers (FQHCs) providing primary, dental, pediatric, OB-GYN, and behavioral health care to over 245,000 patients annually.
