A health services network in Westchester is currently seeking a LMSW or LCSW for a great Case Management opportunity with their Emergency Room. Responsibilities: The Social Worker will: Provides Social Work services to patients and families including obtaining, analyzing and evaluating data relating to medical psycho-social programs; Formulates and carries out treatment plans; initiates and /or follows through in a cooperative effort with other members of the home care interdisciplinary teams to enable patients to be maintained safely in the community or discharged to an appropriate facility. Qualifications: 2+ years of experience in an Acute Care, Hospital, and/or a related setting Master of Social Work Degree from a School of Social Work accredited by the Council of Social work Education Licensed Social Worker (LMSW/LCSW) approved by the State Education Department of the University of the State of New York NYS SCREEN Certification within 3 months of hire dat Desired Skills: Emergency Room experience
