A health services organization in New York City is currently seeking a new Social Worker to join their growing Transitional Living Residence Department at one of their mental health facilities in Manhattan. Responsibilities: The Social Worker will:
- Coordinate the discharge of residents, tracking progress toward discharge.
- Provide ongoing individual and group meetings
- Coordination of residents' benefits, TSOCR rent, PNA, state Medicaid.
- Track / apply for residents birth certificates, social security cards, and resident alien cards.
- Prepare required documentation including but not limited to Service Plans, Service Plan Reviews, Admission Notes, Discharge Summaries, MHARS 725s/ 116s, and monthly Progress Notes
- Attend the Social Work Department meetings, bi-monthly teleconferences with, discharge conferences, case conferences and individual and group supervision
- Escort residents to housing interviews/tours and admissions to new residences, as needed
- Coordinate and follow up on SSI/D benefits as well as State and community Medicaid applications (including completion of applications where needed)
- Perform other duties, as needed
Qualifications:
- Bachelor of Social Work, Master of Social Work or a related Human Services Field
- Prior experience working in a Mental Health and/or Social Services setting
Desired Skills:
- Active, unrestricted NYS Licensed Master of Social Work (LMSW) or Licensed Clinical Social Work (LCSW), or Licensed Mental Health Counselor (LMHC)
