This position requires a dedicated professional with at least one year of agency home health experience and proficiency in Oasis and Homecare Homebase.Key Responsibilities:Provide high-quality care to patients in their home settingComplete accurate and timely documentation using Oasis and Homecare HomebaseCollaborate with a multidisciplinary team to ensure optimal patient outcomesEnsure compliance with all regulations and company policiesSchedule:Full-Time, Monday through Friday, 8:00 AM - 5:00 PMTypically on-call one weekend per monthCompensation:Mileage reimbursement at $0.48 per visitHourly rate is negotiable based on experience and territory budgetIf you are passionate about providing exceptional patient care and meet the qualifications above, we encourage you to apply today!
