Job Title: Community Nurse 12 HOURS WEEKLY- PART TIME 1 YEAR CONTRACT Minimum 3 years' experience in community, public, HOME HEALTH, or HOSPICE • Active Vermont RN License • Valid Driver's License • Current CPR/First Aid Certification • BS in Nursing (Preferred) The focus of the Community Nurse is on home visiting, teaching, coaching, and integration of services, as wellas prevention of disease and promotion of health, for individuals and the community of Chelsea.
