Must have experience with Certified Home Health Agencies (CHHA)RESPONSIBILITIES - not limited to:- Assesses and charts observations of the patient's condition at each visit.- Completes evaluation tasks, including reviewing medication, vital signs, andcomplete head to toe assessment.- Administers skilled nursing treatments and interventions including but not limited toat-home IV infusion, wound assessment, wound care, and education aimed atpromoting and maintaining patient independence.- Revises plan of treatments and discharge plan in conjunction with registered nursecase manager, patient, family, physician, care delivery team, and third party payorsincluding obtaining medical orders and approval of medical services.- Initiates appropriate preventive and rehabilitative strategies.- Consults with and educates the patient, family and other team members regardingdisease process, self-care techniques and preventive strategies.QUALIFICATIONS:1. New York State Licensed RN2. One year of experience as a clinical nurse in a hospital or other direct patient careclinical setting required3. Strong critical thinking skills, the ability to follow standards/procedures, and makeappropriate clinical judgments to provide safe and effective client care4. Ability to work independently with minimal supervision, adapt to change, andmanage a variety of assignments while establishing priorities and ensuring thenecessary duties are completed5. Strong interviewing, clinical assessment and communication skills6. Ability to exercise initiative and independent judgment7. Ability to build and maintain professional relationships8. Current NYS driver's license
