Job Details:

• Coverage: Ongoing

• Infectious Disease Clinician needed for small outpatient clinic; Seeing about 10-14 outpatients per day and monitoring/ordering their treatments

• Positions Open: 1

• Shift: M-F 8:30a - 5p (Clinic - 1 Day no contact day); Call 8a handoff (weekends F/SA/Sun)

• Setting: attached to hospital, but outpatient clinic

• Days per month: M-F

• Required Procedures: in office dressing changes/wound care

• EMR: CERNER

• Support Staff: 1 MA, 1 front office

• 7-12 patients per day

• Travel Preferences: Preferred Lodging: Preferred hotels Air Travel: Fly into Sky Harbor International in Phoenix and use rental car to drive 100 miles to Prescott. Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation

• Credentialing Information: Timeframe: 30-90 days

• Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? - No

• First day info- orientation to the clinic space and EMR training, on first day of patients Cerner support staff is in clinic to answer any questions

Requirements:

• Active Unrestricted AZ license

• Board Certified/Board Eligible

• Residency/Fellowship – Required

• Other Certifications – DEA

• Availability at time of name clear – Required

• Must have clean malpractice
