Job Details:

• Coverage: Ongoing

• MD/DO OBGYN

• Positions Open: 2

• Shift Availability: Mon-Friday 9a-5p

• Setting: Outpatient, Clinic Trauma Level II IFTE: 2.0

• Required Procedures/Job Duties: General OB/GYN procedures & job duties: Candidates will need to see patients in clinic

• EMR: Epic

• Support Staff: MA, and Sonographer in the clinic

• Census/patient population: Adult, Adolescent, Geriatric

• How many patients are seen per clinic day? 10-12 patients per day on a full day.

• EMR Required Training: Module (learn lessons) – Must be completed prior to virtual EMR training to attend the training.EMR Virtual training – Only able to attend if Modules are completed before training. Provider is required to schedule and complete their virtual EMR training prior to their first day, to avoid missing their first shift.

• Travel Preferences: Preferred Lodging: N/A- use standard GSA rate; Air Travel: Air travel okay

• Credentialing Information: Timeframe: 60-120 days

• Credentialing with other facilities? YES

• Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? Yes

Requirements:

• Active Unrestricted UT license Required or IMLC (LOQ in hand at time of submission)

• Board Certified OBGYN - required

• Active BLS or PALS, Controlled substance - Required for credentialing

• Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)

• Clean Malpractice - Required

• Clean Background - Required

• Must include self inquiry NPDB report at time of name clear.
