Provides palliative care and consultations as a partner in outpatient and inpatient care settings. Functions as a member of the palliative team to provide palliative services to patients with serious illness and their families. Assesses physical, psychological, emotional, spiritual and social needs of the patient and family. Makes recommendations for and prescribes medications and other therapeutic forms of treatment in an effort to mitigate symptoms related to serious illness. Documents visits and discussions using an EMR system. Communicates findings and recommendations to the care team, including consulting and attending physicians. Participates in the discharge planning process to assure safe transitions across the care continuum. Keeps current on issues involving goals of care conversations, Advance Health Care Directives and DMOST.
