Practice Setting: Office/Clinic, In house OB hospitalist, and OB GYN call coverageReason for Coverage: Very limited number of board certified OB GYN's who can take the call and 1 provider going on leaveSchedule: 12 or 24 hour blocks when in houseCall Schedule: Call coverage for OB GYN is 1 week minimum per monthTotal Number of Beds: 12 LDRs; 300 births per monthAverage Number of Scheduled Patients Per Day: 25 in office practiceNumber of Physicians in Practice: 2Will Other OB/GYNs Provide Coverage During Locums Coverage Dates (List Names): YesHow Many APPs Provide Coverage: 5Number of Exam Rooms: 4Number of Exam Rooms: 4Onsite Lab? • Low Risk Pregnancy (i.e. History of Twins, Toxemia, Pyelonephritis, etc.)
• High Risk Pregnancy (i.e. with Toxemia, Diabetes, Class II or Higher Cardiacs, Chronic Lung Problems, Chronic Renal Disease, etc.)
• FHR Interpretation - NST/CST/FSE
• OB ultrasounds - Performance of Level I/Screening (Viability)

Labor and Delivery Required Procedures:

• Routine Delivery -Vaginal, Low Forceps, Decision-Making for C-Section
• Complicated Delivery -C-Section, Breech, Multiple Births, Abruptions, etc.
• VBAC's
• Repair of 3rd & 4th Degree Lacerations

Gynecology Required Procedures:

• Office/Minor Gynecology -IUD, Cystoscopy, Colposcopy, Cryosurgery, LEEP, etc.
• Surgical Gynecology - Open
• Laparoscopic Diagnostic/Minor, Only-Tubal Ligation, LOA's, Chromal Tubation
• Operative/Major (Includes Diagnostic) - Ectopic Pregnancy, Ovary Removal, Cystectomy, Extensive LOA's
