Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. We hire people that are skilled and passionate to meet our mission to end homelessness through outcome-based strategies which support personal and community transformation.

The RCP Logistics Assistant works under the supervision of the RCP Logistics Supervisor to provide logistical support for program operations through coordination and implementation of many of the non-clinical needs of participants in the program. Principal responsibilities include cleaning rooms for the next admission after a participant has been discharged, packing up and moving client property according to established protocols, assisting with management of durable medical equipment and program supply storage areas, ordering and picking up program supplies and food, coordinating the food box program and maintaining the food pantry. The team is focused on working with partner hospitals and referring agencies to provide for the acute and ongoing medical care for adults facing homelessness upon discharge from the hospital. The multidisciplinary team coordinates health care through the Blackburn Center, providing short-term supportive housing, crisis management, referral to appropriate substance abuse, physical health, and mental health treatment, access to benefits and entitlements, appropriate and affordable housing, and further provide pathways to self-sufficiency.

Compensation: The pay range for this position starts at $18.30/hour

Schedule: Tuesday-Saturday 8:00am-4:30pm

MINIMUM QUALIFICATIONS:

  • High School Diploma or GED required.
  • A current driver's license is strongly preferred for this role. You may also be asked to pass a DMV background check and be designated an acceptable driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
  • Education and experience sufficient to succeed, carry out responsibilities, and demonstrate the skills and abilities as described above with at least one year experience working with socially complex populations.
  • Must have an understanding of the causes of homelessness, the medical and psychosocial sequelae of homelessness, and the importance of systems and processes used to help clients secure and maintain stable housing.
  • Must have Healthcare Provider CPR/First Aid certification prior to start.
  • Must be able to be trained in the use of CCC database programs CCCER and Centricity.
  • Must have non-violent de-escalation training (or complete within 60 days of hire.)
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • Must have a high tolerance for ambiguity and change.
  • Must be able to work efficiently in a very fast paced environment.
  • Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
  • Must pass a pre-employment drug screen, TB test, and DHS background check for work at Central City Concern and to obtain ID-Badge at Partner Hospitals.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Under the direction of the Logistics Supervisor, clean participants' rooms that have been vacated and prepare the room for the next admission.
  • Coordinate logistical needs required for the day to day operations of program. Maintain the RCP food pantry, donation storage, and supply areas in a clean and orderly manner.
  • Coordinate distribution of food.
  • Coordinate laundry and linen needs.
  • Transport patients to and from appointments and other resources; assist with moving property to a new location in a CCC vehicle as needed.
  • Assist with routine checks of program equipment and vehicles to keep items in good working order.
  • Carry a company mobile phone for heavy use at work.
  • Research information needed to complete projects.
  • Drive to various locations to pick up supplies and/or make resource contacts.
  • Attend all required trainings and meetings.
  • Complete a variety of computer-based tasks such as; ordering and receiving supplies; tracking inventory; email correspondence with vendors and donation sources.
  • Maintain notes and tracking information about patients enrolled in the program.
  • Assist multi-disciplinary team members with program operations as assigned.
  • Assist multi-disciplinary team in coordinating and implementing participants' discharge and transition from RCP.
  • Demonstrate personal attentiveness, responsiveness, and prompt follow-up to all external and internal stakeholders.
  • Assist with resolving patient complaints and providing good customer service throughout the program.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Attend all mandatory CCC trainings in a timely manner.
  • Perform other duties as assigned.
