Job Title: “Long Term Care Nurse Specialist”

Location: Brooklyn, NY USA / Bronx, NY USA / Manhattan, NY USA

Duration: 03 Months of Contract (Possible Extension)

Shift Time: 8:30 AM - 5:00 PM position (7.5 hours daily). Overtime may be required.

Day to Day Responsibilities:

• RN must prep for the visit the morning of or afternoon before (Review of EMR and previous assessments)
• Be prepared to use critical thinking and problem-solving skills.
• Travel to the members home to conduct the visit.
• Accurately document according to the provided documentation template which is a minimum of what is expected.
• Review all documentation and utilize Lenavi to identify inconsistencies that need to be corrected or validated.
• Complete and load supplemental tools and documents.
• Each day varies, nurse needs to be flexible with schedule and process changes, critical thinking and problem solving, prepared for travel, weather variability.
• Ability to be non-judgmental when visiting members' homes and experiencing adverse events.

Describe the performance expectations/metrics for this individual and their team:

• Complete 2-5 visits depending on the type of visit assigned.
• Visits have different time allotments assigned based on what is required during the visit.

What previous job titles or background work will in this role?

• Individuals that have MLTC, MAP or Maximus experience
• UAS RN

Required Skills/Experience:

• MLTC, MAP or Maximus experience
• Flexibility
• Excellent written and verbal communication
• Technology: outlook, Excel, Microsoft

Education Requirement: AS

Required Certifications: RN, NY State Driver's License
