Position Summary: The Nurse Practitioner (NP) will work in collaboration with the Skilled Nursing Facility interdisciplinary team to coordinate and support the participant's functional, clinical and psychological needs. This role will work as the liaison between the resident, the physician and the nursing staff to develop and implement the plan of care including patient-centered goal development, communicating changes in resident condition and facilitating preventive and treatment-based services. Job Responsibilities:
Reviews the patient's past medical history every other month and formulates a comprehensive and complete diagnostic list of current and past medical conditions using clinical knowledge and judgment and the findings of his/her assessment. The NP is responsible for ensuring that all such complete, accurate and specific diagnosis codes will be documented in the resident's medical record. Reviews the patient's current symptoms, exacerbation of problems that were previously controlled and identifies active diagnoses and chronic problems or conditions to be used for active medical management and treatment. Conduct in person visits to the residents every other month and as needed to cultivate appropriate lines of communication around patient care delivery. Conduct routine clinical assessments every other month on all long-term care residents which reflects an understanding of the complexity and multiple co morbidities of the frail and/or elderly. Assess and manage residents with acute change in condition in a timely manner. Adhere to facility protocols and expectations regarding timely provision of required clinical documentation. Monitors and schedules patient care visits from review of Patient care dashboard . Reviews daily change in condition reports and schedules follow up visits as needed. Completes and submits daily billing log, includes CPT codes and ICD 10 codes for resident care visits. Experience working with Microsoft Office and working in electronic documentation EHR platforms.
