Marquis Certified Home Care (CHHA) is in search of a Licensed Practical Nurse (LPN) to care for our members/clients/patients residing in an assisted living facility in Amsterdam, NY for Home Care. Qualified candidates will have a NY LPN license; will have a minimum of 1 year of Home Health, Acute Care, Rehab, or Medical-Surgical nursing experience - CHHA/LTHHCP experience preferred. LICENSED PRACTICAL NURSE: Full-Time 10 Hour Shifts Rotating Schedule - Week 1 (M, W, F) Week 2 (T, Th, S, S) Location: Amsterdam, NY LICENSED PRACTICAL NURSE COMPENSATION: Salary Negotiable Perks :

What you will do:

• Provide quality skilled nursing care to residents, care may include but is not limited to those with diabetes, wounds, and injectable medications.
• Document accurately and efficiently in EMR.
• Work collaboratively with the RN of both the CHHA and the RN in the facility as well as communicating with other staff as needed including administrators, providers, and therapists.
• Reporting changes in condition and other findings as per agency protocol.
• Participate in agency's quality assurance program.
• Other tasks as assigned.

Ideal Candidate:

• Active NYS Licensed Practical Nurse Licensure/Registration
• Active NYS Driver License, CPR/BLS Card
• One year of Licensed Practical Nurse experience is preferred, however, new graduates are welcome to apply
• Previous homecare experience and/or acute care or rehab experience is a plus
