Shift: Full-Time (M-F) + rotating weekends
Hours: Day (7am-3pm), Evening (3pm-11pm), Night (11pm-7am)

Be a graduate of an approved school of nursingBe licensed and currently registered to practice as a Licensed Practical Nurse in the State of New York.Have the ability to observe and identify the resident's needs, report to the RN/Supervisor and collaborate with the RN in making clinical decisionsPrioritize and coordinate activities of the nursing unitBe an excellent problem solver and have strong decision-making skillsHave demonstrated leadership abilitiesHave prior experience working in a skilled nursing facility (preferred, but not required)Experience with SigmaCare (preferred, but not required)Required Qualifications:
An active LPN license for New York StateUnexpired verification of US CitizenshipUp-to-date physical examination (within the past 12 months)MMR titers and immunizationsCPR certificationMalpractice insurance(2) professional referencesAbout the position:
The LPN functions under the direction and the supervision of the RN/charge nurse and the Nursing Supervisor. Responsibilities include but are not limited to the following:
The LPN is the leader of the unit coordinating the resident's care in collaboration with the CNAs, physician, and other disciplines through assessment, under the guidance of the RN/Supervisor, and involvement with the resident and family.Leads and communicates with the team in the planning and implementation of care and thorough documentation of the same in the EMR.He/she is proficient, or working towards proficiency, in wound care, ostomy care, foley catheter insertion and care, medication administration through all routes including G-tube and IVs, trach care and suctioning.Through keen observation of any actual and potential changes in condition, the nurse together with the other members of the team will respond and react timely and according to protocol to prevent re-hospitalizations and potential emergency situations. In the face of an emergency situation, the nurse responds efficiently and calmly to initiate the rapid response or emergency response team as appropriate.Learn more about life at Cobble Hill:
