JOB DUTIES/RESPONSIBILITIES: (Adhere to scope of practice for Licensed Practical Nursing per New York State regulatory guidelines.)1. Adhere to agency policy and procedures.2. Maintains confidentiality of all agency related and client information.3. Attends and participates in staff meetings, in-services, Quality Improvement activities and other meetings as requested.4. Assumes duties and responsibilities as assigned by immediate supervisor or designee.5. Provides direct client care according to the therapeutic plan under the direct supervision of the Nursing Field Supervisor/Director of Patient Services.6. Provides appropriate information to assist the RN in the preparation and maintenance of an effective plan of care.7. Reinforces instruction given to the client/family by the RN or physician.8. May administer prescribed medications to clients under specific policy of the agency.9. Performs selected prescribed treatments and nursing measures for the client in home care after demonstrating skill to the Nursing Field Supervisor or Director of Patient Services.10. Notifies the RN of significant findings or changes in the client's physical and/or mental condition.11. Documents on official records reflecting the client's condition and response to medications, treatments and nursing tasks.12. Adherence to LPN scope of practice limitations, i.e., inability to perform nursing assessments, but may report to an agency RN on any patient observations noted.13. Other related duties as assigned.EDUCATIONAL REQUIREMENTS: 1. Graduate from an accredited School of Practical Nursing.2. Current valid New York State license and registration as a Practical Nurse.3. Minimum of one (1) year medical/surgical experience, preferred.ADDITIONAL REQUIREMENTS: 1. Ability to comprehend and communicate information both verbally and in writing.2. Ability to establish and maintain effective interpersonal relationships.3. Ability to set priorities and organize work.4. Ability to relate to a diverse and multi-cultural population.5. Ability to be flexible and work with minimal supervision.6. Ability to function autonomously, to make decision, to problem solve, and to manage emergency situations in the home.7. Ability to successfully complete a Competency and Proficiency Test and skills evaluation appropriate for the position upon hiring and annually thereafter.8. Pass a physical examination and immunization requirements after being offered employment and a health assessment annually thereafter.9. Possess a valid driver's license and has a car available for work use.10. Able to lift 50-pounds, be able to sit, stand, bend, stoop, kneel, pull, climb, reach, lift and perform repetitive movements of the upper extremities.
