DescriptionYour Experience Matters – New Higher Salaries Based on ExperienceHourly pay range: $23 - $26As directed by the RN, assists with the monitoring and coordination of the delivery of health care services for individuals with developmental disabilities who are living in community based homes.ESSENTIAL FUNCTIONS/RESPONSIBILITIES:Assists with monitoring and coordinating the delivery of preventive and routine health care for individuals with developmental disabilities.Adheres to and assists with ensuring facility compliance with all Physician, NP, PA, etc. orders.Completes required nursing care documentation promptly.Assists with coordinating medical program needs with community services, medical providers and other related agencies.Assists with ensuring adequate scheduling and completion of health related exams, appointments, laboratory/diagnostic testing as ordered or required.Safely transports individuals to and from appointments if necessary.Assists with the monitoring of medication administration in the facility.Administers medications, treatments and supplements as assigned.Communicates issues and carries out all directives of overseeing RN and Nursing Administration.Participates as an active member of the Interdisciplinary Team.Maintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure.Maintains communication with Medical Liaison, Site Management Team, Service Coordinator and other staff as necessary.Participates in various nursing workgroups as assigned.Provides nursing coverage to all People Inc programs as directed by Nursing Administration.Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).Complies with all agency policies and procedures.Other duties as required by Nursing Administration.MINIMUM QUALIFICATIONSCurrent NYS licensure to practice as a Licensed Practical Nurse.Valid Drivers License that meets agency policy.Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.Lifting requirements of 35 lbs.Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan.Ability to move, reposition, and place individuals in pieces of equipment.Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.CPR certification required.Why People Inc.?When you join the People Inc. team, you can make a difference in the lives of people receiving services while also receiving great benefits (generous PTO, PTO buyback, affordable insurance, tuition reimbursement, career mentoring, shift incentives and more), flexibility and the opportunity to help others move closer to their life goals and dreams.
