A health services network on Long Island is currently seeking a new Licensed Practical Nurse (LPN) to join their growing team at one of their facilities in Nassau County.About the Opportunity:
Responsibilities:The Licensed Practical Nurse (LPN) will:
- Assist in data collection on individuals within assigned caseload
- Contribute to the development of individual's plans and assist in identifying areas of need.
- Assist in observation and evaluation of nursing care administered by Direct Support Professionals
- Ensure that all required documentation is completed /processed appropriately and in a timely manner
- Assist in maintaining effective communication with staff, physicians and Registered Nurse.
- Review physician summary forms to ensure information is complete, accurate, and documented in the individual's medical record
- Ensure Emergency Room instructions and/or treatment is followed up and documented accordingly
- Attend medical appointments when necessary and/or requested by the Supervisor
- Assist with ensuring accuracy of MARs for use at beginning of the month
- Ensure all individual's self-preservation through implementation of the fire evacuation plan when participating in scheduled fire drills
- Perform other duties, as assigned
Qualifications:
- Graduate of an accredited Practical Nursing program
- NYS Licensed Practical Nurse (LPN)
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bachelor's Degree
- Experience working with the differently-abled population
- Knowledge of OPWDD State and Federal regulations
