Advanced Rehabilitation & Healthcare of Vernon -

Job Purpose:

To provide nursing care to residents as prescribed by the physician and in accordance with standards

of nursing practices and regulations.

Essential job Duties and Responsibilities:

Nursing Care:

Prepare and review medical records and nurses notes, and assess residents to observe and evaluate their physical and emotional status. Monitor resident activity, evaluate nursing care in accordance with professional standards, and provide nursing care according to physicians orders, care plans, established standards and facility policies. Coordinate and assist or prepare for new admissions,

transfers, and discharge of residents. Transcribe physicians telephone orders. Communicate with Residents, family members, other interdisciplinary team members and management regarding resident status.

Drug Administration and Documentation:

Prepare and administer medications as ordered by the physician; in accordance with nursing

standards and facility policies. Sign, date and perform all charting and record keeping in accordance with established policies and procedures.

Direct Patient Care:

Lead and monitor licensed and other professionals. May schedule job assignments and set priorities. Assist and coach as needed. Provide feedback as to their effectiveness; may serve as team leader in

this function.

Resident Rights:

Understand, comply with and promote all rules regarding Residents rights.

Other Job Functions:

Care Plans:

Assist and participate in developing and implementing a written care plan for individual residents that

addresses the needs of the resident.

Staff Development:

Participate in in-service training, assist in implementing orientation programs and job skills training, maintains professional competence

Safety and Sanitation:

Monitor environment and care practices of nursing personnel to ensure compliance of established safety, fire and disaster, infection control and ensure all other departmental policies and procedures are followed; and to ensure cleanliness and safety of work and treatment areas

Other Duties:

Physical and Sensory Requirements (with or without the aid of mechanical device):

Works in a climate-controlled and well-lighted environment.. Works in an office and throughout the building daily. Has Category 1 risk of exposure to infectious diseases, but receives training in handwashing practices and Universal Precautions and has access to personal protective equipment if direct exposure is anticipated. Is subject to interruptions, noise, and odors daily

Walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells, tastes and temperatures, ability to communicate with residents and employees, ability to understand and follow training, ability to instruct staff in service education, and the ability to remain calm under stress.

Qualifications:

Must be a Registered Nurse (RN) in good standing and currently licensed by the State. Experience as a Charge Nurse or supervisory experience preferred.

Must be CPR Certified.

Ability to convey, explain, or interpret complex specialized information related to long term care rules and regulations. Hence strong verbal and written communication skills are required. Ability to complete continuing education hours as required by the State.

Ability to build and maintain relationships, coordinate plans, distribute information and develop a mutual knowledge base for effective implementation of state and federal rules and regulations. Must be able to relate positively and favorably to residents, families, and government regulators. Must be capable of maintaining regular attendance.
