Job Title: Manager, Home and Community Care

Locations: Saanich or Sidney, British Columbia

Job Type: Full-Time

Salary Range: $95,000 - $105,000

As the new leader of one of Bayshore's local Neighborhood Health Teams (NHTs), you will collaborate with an integrated regional team to oversee a range of in-home care services, manage client service programs, and oversee all team operations.

DUTIES & RESPONSIBILITIES

The responsibilities of this role include, but are not limited to:

* Direct and monitor the delivery of all in-home services and daily functions of field staff providing access to clinical advice, support, and resources.

* Lead the sales process and business development, client acquisition strategy and marketing activities for your Neighborhood Health Team (NHT).

* Build and sustain strategic and collaborative relationships with current, new and prospective partners and community organizations.

* Assess and document community needs while developing business and funding proposals to meet emerging opportunities.

* Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals, and continue to resolve client's challenges by offering services, products, advice, and community resources.

* Hire and train all NHT staff members, including training and orientation on all services, products, and pricing; supervise the day-to-day performance of the team; ensure exceptional interactions with prospective and existing clients; and provide ongoing coaching and performance management.

* Assist with the delivery of in-home services, as required.

CANDIDATES MUST HAVE:

* A current and valid Registered Nurse (RN) registration with BCCNM with a minimum of five (5) years of recent experience, preferably in a community setting, and at least two (2) years managing others.

* Current and valid CPR certification.

* Experience in community work and working with community partners in a collaborative way.

* Experience with business development and in engaging with municipal and provincial groups.

* A reliable vehicle and willingness to travel to visit clients within the community (roughly 60% in the field, vehicle allowance is provided).

* A solid knowledge of the principles, practices, and methods of program development, implementation, and evaluation.
