Job Type Full-timeDescriptionSUMMARY: Organize and coordinate the MAT programs at all AxessPointe locations. Provide administrative support to the MAT Clinical Lead physician and providers as needed. Contribute to MAT patients' optimal recovery experience and outcomes by facilitating retention in care and adherence to treatment plans. Work collaboratively with AxessPointe's behavioral health, medical and dental teams to enhance the continuum of recovery-oriented care based on patients' needs. Prioritize patient care with timely and accurate communication. Contribute to an AxessPointe/FCS culture that is free of stigma related to substance use disorders; promote awareness of successful recovery strategies and behaviors. Work collaboratively with Peer Recovery Specialist to support the Harm Reduction Program (SafePointe). Seek and apply for grant funding. Develop and maintain relationships with partner agencies and funders and attend community events as appropriate. Ensure compliance with regulations and adherence to best practices.SUPERVISOR: PhysicianDUTIES AND RESPONSIBILITIES:
  • Organize and lead monthly and quarterly meetings of the MAT program team, providing updates, program improvements, and problem-solving
  • Collaborate with Marketing staff to develop and distribute outreach materials
  • Track and report patient volume and program outcomes; produce reports for internal use, external funders, and new grant funding opportunities
  • Complete required documentation for various grants supporting both treatment and harm reduction efforts including surveys/interviews and quarterly progress reports.
  • Coordinate medical provider schedules to ensure access to MAT care at all participating AxessPointe sites and monitor provider schedules to ensure patients are scheduled appropriately.
  • Receive initial inquiries about MAT services and provide informal education related to MAT options
  • Schedule all new MAT patients for their induction appointment and complete brief SUD/SDOH screening.
  • Complete timely and accurate documentation regarding any patient interaction in the electronic health record (ECW).
  • Provide written and verbal updates to medical and behavioral health providers to ensure patient is receiving consistent and comprehensive care.
  • Meet with patients in person and over the phone as needed to address barriers to treatment; assist patients in navigating community resources related to SDOH including housing, transportation, food, and any other needs.
  • Assist patients in obtaining health insurance when appropriate.
  • Coordinate internal referrals to primary care, dental care, IOP and behavioral health.
  • Coordinate with referral staff to facilitate external referrals to higher levels of care (residential, inpatient, detox), when appropriate.
  • Communicate with criminal justice partners and specifically the drug court programs, to coordinate patient care and provide weekly updates to drug court staff.
  • Work collaboratively with the Peer Recovery Specialist to support the Harm Reduction Program (SafePointe) including the distribution of naloxone (Narcan), sterile syringes, and other harm reduction strategies.
  • Attend internal and external meetings as appropriate, representing and promoting AxessPointe's MAT program and other services
  • Provide resources to clinical staff as needed for optimal care (educational materials, induction folders, sobriety recognition items)

RequirementsREQUIREMENTS:
  • Knowledge and understanding of the range of services available to individuals with substance use disorders.
  • Skills in crisis intervention and de-escalation.
  • The ability to communicate effectively verbally and in writing.
  • Attention to detail and excellent organizational skills.
  • The ability to track program outcomes and produce analytical reports
  • The ability to establish and maintain effective relationships with clients/patients
  • Ability to function as part of an organization by working with others and by adhering to established policies and procedures.
  • Must be knowledgeable of and adhere to all pertinent federal, state, and local laws, standards of the Ohio Department of Mental Health, applicable privacy requirements, and appropriate professional ethical standards.
  • Ability to meet agency requirements for criminal background check.
  • Must possess a valid Ohio driver's license in accordance with County policy and must maintain motor vehicle liability insurance, must maintain continuing eligibility under the existing County driver eligibility standards.
MINIMUM QUALIFICATIONS:
  • Bachelor's degree in social work, Health Education, Public Health or related field.
  • 2 years' experience in a social service, community health, or clinical setting
LICENSES OR CERTIFICATIONS REQUIRED:
  • Valid Ohio driver's license
