Responsibilities:
ESSENTIAL DUTIES:
- Models a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
- Ensure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
- Knowledge of Medicare pre-admissions for qualifying diagnosis and coverage eligibility
- Obtains/ maintains physician certificates for each Medicare resident
- Maintain good attendance record
- Maintains MDS schedule
- Completes MDS 0 for Medicare residents
- Assures that all MDS information is completed and transmitted per policy, and is compliant with all regulations and forwarded to billing office
- Tracks Medicare resident benefits days, validates daily Medicare census and coordinates this information with accounting representative and therapy representative, to ensure accurate billing (if applicable)
- Assures all quarterly resident assessments are completed
- Assures all care plans completed and updated as needed
- Coordinates daily PPS meetings and weekly Medicare meetings
- Reviews nursing documentation of residents and provides guidance/training as needed, to ensure charting is appropriate and meets coverage guidelines
- Responsible for completing case management to ensure adequate documentation for reimbursement
- Coordinates discharge of residents
- Works with Clinical Liaison to verify appropriateness of admission and admission diagnosis codes
- Maintains weekly communication with insurance case manager to receive approval for resident treatment and communicate with physician on insurance resident admission for timeliness of physician visits
- Communicates with rehab department on routine basis, to discuss therapy services
- Communicates with Life Enrichment and Social Services to coordinate assessment in a timely manner
- Other duties as assigned by Director of Nursing
Qualifications:
MINIMUM QUALIFICATIONS:
- Valid Texas LVN/RN license
- AANAC RAC-CT Certification
- In depth knowledge of MDS
- Minimum of 2 years experience with RAI process/MDS/PPS/Medicaid CMI in a Medicare/Long Term Care setting
- Experience with electronic medical charting
- Demonstrated MDS application skills
- Knowledge of SNF, Medicare documentation and reimbursement guidelines
- Demonstrated MDS application skills
- Demonstrated computer competencies
- Knowledge of rehabilitation theory and application
- Knowledge of current professional nursing theory and practice
- Ability to read, write and effectively communicate in English
- Excellent verbal and written communication skills
- Knowledge of restorative nursing care practices
- Demonstrated supervisory and managerial skills
- Ability to maintain a stable disposition in performing work in association with staff and residents
- Ability to maintain a high level of performance in an environment of frequent demands and changes
- Ability to perform Essential Duties as outlined below
- Genuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients
- Ability to perform work tasks within the Physical Demand Requirements as outlined below
