Join our energetic team at Maple Lawn Senior Care! Our MDS Coordinator position is a full-time position that offers employee benefits. This position is Monday through Friday 8:00am-4:30pm, weekends based on RN on-call schedule. Will be responsible to fluctuate office hours with floor hours based on census, admissions, discharges. This position includes being on the RN on-call schedule. Position may include other duties based on organizational needs, if necessary. Job Summary: Responsible to ensure compliance with RAI and MDS processes. Must be able to communicate and work well with other leadership staff members that assist with MDS sections. Develop a good rapport with all other departments to ensure teamwork when developing resident's plan of care. Includes the interviewing process, inputting, and evaluation of each resident's MDS and overall functionalities. Reviews and analyzes nursing documentation to code into MDS. Creates, reviews, and updates all resident's care plans with Director of Nursing and other nursing staff. Responsible for attending weekly Care Conferences to review resident's overall wellbeing and changes. Completes comprehensive assessments as needed for admissions, discharges, quarterly schedules, etc. Ensures CNA's and floor staff are accurately documenting for proper MDS coding. Assists with insurance prior-authorizations before or upon admission. Responsible to update insurance companies, or care coordinators, to ensure payment. Be active in the discharge or transfer process of residents. Communication with family and resident is highly important. Assists the Director of Nursing to create and implement an effective onboarding process for new hires. Must be comfortable directing and correcting staff when noncompliance is indicated. Preform audits when necessary to confirm compliance or monitor for areas of improvement. Expected to be part of the Interdisciplinary Team and Quality Assurance Committee. Must show knowledge of technology such as computers, Excel, Word, etc. Encourage other disciplines to bring forward for discussion of the IDT team concerns regarding significant changes so the need to significant change status assessments will be done as per policy. Committee Functions Maintains a positive attitude and create a positive attitude while at work. Must be flexible and able to change between tasks at a moments notice. Must be reliable and dependable. Maintains a clean, professional manner. Assumes accountability for compliance with RAI processes, federal, state, and local regulations. Maintains confidentiality throughout. Qualifications: Must possess an active MN Registered Nurse License. Three years of experience preferred. If not MDS certified, will need to complete the proper training and education.
