• You Will Play a Critical Role: As an LPN /RN MDS Coordinator, you'll conduct and coordinate resident assessments, ensuring compliance with federal, state, and local standards.
• Team Collaboration: Work closely with the Interdisciplinary Care Plan Team to develop comprehensive care plans for each resident.

LPN/RN MDS Responsibilities Include:

• Resident Assessment Development: Conduct and coordinate Resident Assessment Instruments (RAIs) in line with regulations and guidelines.
• Supportive Collaboration: Collaborate with the Care Plan Team to craft personalized resident care plans.
• Evaluation and Reporting: Assess resident conditions, ensuring special assessments are conducted when necessary, and provide necessary data for the Facility's Quality Assurance Program.

LPN/RN MDS Qualifications We Seek:

• Active LPN/RN MO License: Current and in good standing.
• MDS Coordinator Experience: Prior experience in a similar role.
• Regulatory Knowledge: Familiarity with Medicare/Medicaid regulations and benefit guidelines.
