MDS - Wildwood Health and Rehab : Job Details

MDS CoordinatorDuties:- Coordinate and oversee the Minimum Data Set (MDS) process for accurate and timely completion- Ensure compliance with regulatory standards and guidelines related to MDS assessments- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments- Conduct comprehensive assessments of residents' medical, social, and psychological needs- Develop individualized care plans based on assessment findings and resident goals- Monitor and update care plans as needed to reflect changes in residents' conditions- Provide education and training to staff regarding MDS assessments and documentation requirements- Participate in discharge planning and ensure smooth transitions of care for residentsQualifications:- Bachelor's degree in nursing or related field preferred- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)- Experience with MDS assessments and knowledge of HEDIS standards- Proficiency in using electronic health record systems such as Cerner, Athenahealth, eClinicalWorks, etc.- Strong understanding of primary care, hospice care, and pediatrics- Excellent communication and interpersonal skills- Attention to detail and ability to maintain accurate documentation*Note: This job description is not intended to be all-inclusive.
