Successfully completed the MDS for submission and reimbursement. Coordinates Hospice and Respite services.
- Communicates changes in reimbursement/billing issues and Medicare via the computer and Business Office.
- Provides resources and participates in continuing education/staff development regarding MDS.
- Schedules MDS/IDC within required time frames.
- Assures completion of MDS and RAPs.
- Meets with families and staff to acquire data for MDS completion.
- Notifies sponsors of change(s) to payor source complying with minimal 2-day notice.
- Participates in demand bill process.
- Performs assessments of potential respite applicants.
- Develops, amends respite program policies and procedures.
- Accurate documents assessments.
- Acts as liaison between Nursing Home and Hospice program.
- Other duties as assigned
EducationWhat we need from you
- Completion RN Program, required
- BSN Bachelor's Degree, preferred
All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire.BSN must be completed within 10 years. RN's licensed on or before 12/19/17 are/remain exempt from this requirementExperience
- Minimum one (1) year long term care experience, required
- MDS experience, preferred
Licensure/Certifications
- Current NYS RN license
- MDS certification, preferred
