We do this through a value driven framework of safe and exceptional patient care, accountability, kindness, mentorship, excellence and dedication.The Medical Assistant/Pharmacy Technician will assist Oak Medical providers to enhance patient care and care plan coordination within skilled nursing facilities. The Medical Assistant/Pharmacy Technician is required to work in the field with their assigned providers. The role will require a presence in multiple locations during the week and may be asked to travel to facilities up to an hour from your home. Some work will be remote from home. Excellent communication and customer service skills are a must. The best candidate will be a highly motivated individual able to manage multiple workflows with confidence and minimal supervision in a fast-paced environment.Responsibilities:Ensure that the clinical providers in the assigned building are able to focus on taking care of their patients for the day.Collaborate with provider, patients and families and facility staff to facilitate proper patient management.Ensure check in and out each day with facility staff regarding patients seen, status, care changes etc.Facilitate and host telehealth visits for providers.Building chart templates prior to patient appointments.Review medication list.Maintain patient confidentiality.Qualifications/Education:A minimum of one year experience as a Medical Assistant or Pharmacy TechnicianHigh School DiplomaCertified Medical Assistant (Preferred) or Licensed Pharmacy Technician in the state of WI (Preferred)Driver's license with clean driving record (Required)Reliable vehicle transportation (Required)Flexibility with a growing and changing environmentWhat We Offer:A chance to be part of a growing and emerging cultureMileage reimbursement planPaid time off benefitsHolidaysHealth benefits (medical, dental, vision, life insurance)Short & Long-term disability options401k401k Employer Contribution
