Medical Coordinator/Direct Support ProfessionalJob Category: Direct Service WorkerRequisition Number: MEDIC003372Posted: October 9, 2024Employment Type: Full-TimeLocation: Ozone Park, NY 11416, USARate: $19 USD per hourDescriptionIn collaboration with the RN, ensure that all medical appointments are scheduled in a timely manner and in accordance with regulations.
  • Prepare clinical consultation forms for all medical appointments.
  • Ensure sufficient staff are scheduled for each appointment, in collaboration with the RN and Residence Manager.
  • Accompany individuals and/or staff on medical appointments whenever possible.
  • Review and sign all medical documentation with the RN and/or Residence Manager; file in the individual's medical binder once signed.
  • Ensure all medical appointments and lab work are up to date.
  • Document and implement all physician recommendations.
  • Develop a medical appointment calendar (weekly/monthly, as determined by the RN and Residence Manager).
  • Maintain and obtain AMAP certification.
  • Submit all prescriptions to ChemRx Pharmacy upon receipt; count/record medications upon delivery.
  • Complete Client-Specific Sheet for new medications delivered.
  • Check Medication Cart/Room weekly for expired medications and sufficient supplies for a minimum of five (5) days.
  • Reorder medications when supply is insufficient.
  • Ensure ChemRx Pharmacy receives all original prescriptions.
  • Review and log all control substances; validate count weekly.
  • Discontinue medications as indicated by the prescribing physician, in collaboration with the RN.
  • Maintain a clean and orderly Medication Room/Cart; order medical supplies as needed.
  • Ensure all AMAP signatures are on the Medication Administration Record (MAR) prior to the first administration each month; file MAR documentation monthly.
  • Prepare off-site medication for home visits, trips, etc., in accordance with OPWDD regulations and agency policy.
  • Assist RN in maintaining individuals' medical binders and completing required forms.
  • Complete all paperwork as requested by RN or Resident Manager.
  • Schedule and complete pharmacy reviews quarterly.
  • Monitor Bowel Movement charts daily; report issues to RN and Manager.
  • Perform additional duties as requested by the RN, Residence Manager, Program Coordinator, or Program Director.
  • Assist with morning routines and ADL outcomes for individuals.
  • Assist with dinner preparation if all medical coordinator responsibilities are completed.
  • Support individuals with post-program activities as needed and/or requested.
  • Complete laundry if all medical coordinator responsibilities are fulfilled.
  • Food shopping for dinner if needed and time allows.
  • Possess comprehensive knowledge of OPWDD regulations.
  • Demonstrate strong organizational and interpersonal skills.
  • Exhibit good oral and written communication skills.
