Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work-life balance flexibility!We are seeking a Fee For Service (1-2 days per week) Attending Physician/Medical Director for our Logan Center in Logan, WV. This is a Dual Role Attending Physician/MedicalDirector. It is clinical in nature. Call is minimal as we have a third party telehealth company that assumes most of the call. There is flexibility with the schedule. Must be BC/BE Internal Medicine or Family Medicine with LTC Experience.The Attending Physician works with the Regional Medical Director to promote the delivery of clinical services in a manner that fulfills the AlignMed Partners mission of achieving the highest levels of clinical quality, efficiency, and outcomes. This is accomplished by coordinating with Centers, Regional Management and AlignMed Partners to oversee the implementation, utilization, and outcomes measurement of AlignMed Partners Clinical Programs.The Medical Director is dually accountable to the Center Administrator and the AlignMed Partners Regional Medical Director for clinical care within the Center, accountable to the Center Administrator for delivery of clinical services as stipulated in the Medical Director Agreement, and accountable to the Regional Medical Director for the quality and efficiency of clinical services and outcomes within the Center.Medical Directors partner with the Center Administrator and Director of Nursing, to achieve the highest levels of Center performance in quality, safety, and efficiency. Our Medical Directors share accountability for Center success in the evolving environment of narrow referral networks and value-based reimbursement, providing effective and proactive.RESPONSIBILITIES:Take complete responsibility for patients admitted to their serviceComply with the AlignMed Partners credentialing processVisit assigned patients in a timely fashion according to AlignMed Partners policy and procedureComply with guidelines related to AlignMed Partners initiativesComply with the Rules and Regulations of the Medical StaffComply with the AlignMed Partners Corporate Integrity policiesAssess the customer and ensure that a complete history and physical examination are on the chart within 72 hrs after the admission to the center.Deliver high quality care to assigned patientsWork closely with the Center Medical Director and act to improve the care in the facilityServe as an appropriate role model for other physicians and staffHelp articulate the AlignMed Partners mission to the community and professional organizationsHelp the center establish appropriate relationships, and coordinate transfer and care arrangements with area hospitals and other pertinent institutionsDemonstrate care and compassion to ensure that all internal and external customers consistently receive high quality serviceComply with the AlignMed Partners Time Off and On Call policiesPerform other duties as assignedADHERES TO COMPLIANCE RESPONSIBILITIESComply with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance Process, Standard/Code of Conduct, Federal False Claims Act and HIPAA.Participate in required orientation and training programs.Promptly report concerns and suspected incidences of non-compliance to management or to the Compliance Officer via the Reach Out Line.Cooperate with monitoring and audit functions and investigations.Training and Certification:M.D. or D.O. required.Board Certified in Internal Medicine or Family Medicine preferred, with maintenance of certificationGeriatrics specialization is desirable.Certified Medical Director (CMD) through AMDA is desirable.
