Our interprofessional care teams, which include physicians, nurses, social workers, and patient navigators, provide high-touch and comprehensive care to a small panel of patients. Team collaboration is essential to our intensive, psycho-social model of care which includes assessment, medication management, social management, and behavioral health management.

Job Description

The Market Medical Director is a clinician leader who works in partnership with the Duo General Manager to lead a successful clinical and administrative program in a Duo Market. Together that dyad partnership supports the program through high quality clinical care, responsive and supportive administrative management and proactive in-market relationship building that facilitates care of our teams. As part of this leaders responsibilities, they will both lead our clinical teams in the market (Nurse Practitioners and PAs) and practice themselves with a small panel of high-risk patients. At Duo, we meet patients where they are – whether that's in their home, in a dialysis facility or in the hospital after an admission. It's crucial that our Market Medical Director can role model the practice of in-home care and coach and develop team members who are often not co-located in a clinic setting. On top of that limited clinical role they will have mission-critical leadership responsibilities, which include driving engagement amongst new patients, insuring accurate clinical documentation and measuring and driving differentiated medical cost outcomes. This is an ideal role for a clinician leader who wants to drive outcomes over volume in healthcare and build a team that wraps fully around patients.

Within the dyad partnership the Market Medical Director role specifically includes:

Responsive and supportive administration:

  • Direct oversight of advanced practice providers (APPs)
  • Support and develop for the overall Duo standardized clinical program as it is expressed in the market

High quality clinical care:

  • Holds a small panel of high cost patients and conducts visits with those patients in person and telephonically
  • Works directly with all APPs on new/annual patient assessments and our performance on thoroughly and completely documenting burden of illness
  • Quality performance on targeted HEDIS measures and additional internal Duo quality metrics
  • Medical expense reduction:
  • Oversee the Duo care transitions program inclusive of as needed discussions with hospital/facility based teams and patient visits
  • Lead weekly meetings focusing on transition patients and high cost patients

In-market relationships:

  • Foster stronger relationships with partner clinicians (e.g. PCPs and Nephrologists and others)
  • Identification of high value partners to facilitate our high quality care for the care teams

Program Leadership: Partner with GM to prepare materials and represent the program at:

  • Quarterly provider meetings
  • Internal performance meetings
  • Market staff meetings
  • Business development meetings

Requirements

  • M.D. or D.O. Graduate
  • Board Certification/Eligible in Family Medicine or Internal Medicine Required
  • 5+ years experience in outpatient or home-based practices required
  • Prior experience in value-based care strongly preferred
  • Experience in clinical leadership roles coaching and developing NPs, PAs or MDs preferred
  • Experience utilizing KPIs and data to drive improved cost, quality and clinical outcomes for patients
  • Comfort with an evolving and growing clinical program and/or experience in a start-up environment is helpful
