Empowering People's Independence (EPI), currently has Full time Medical Liaison/ Direct Support Professional position available. This position is responsible for scheduling and coordinating medical, dental, and counseling appointments for the individuals EPI serves.Essential Functions:Maintain the health and well-being of the individuals EPI servesSchedule and coordinate all medical appointments for the individuals EPI. serves including acute appointments - transport and accompany people to all appointmentsNotify circle of supports of all appointments and results of appointmentsEnsure medical provider documents the results of appointments and recommendations on all appropriate form; send copies of consults to appropriate personnelAlert RN and supervisor of any updates needed to IPOP or PONSAssist in obtaining and maintaining needed adaptive equipment/suppliesReport all policy violations to appropriate personnelPromote workplace safetyEducation and Experience:High school graduate or equivalent and one-year experience in the human service fieldCurrent/valid NYS driver's license and reliable transportation required.Team playerFamiliar with the use of email, the internet, and online applications a plus!Qualifications:Maintain a valid New York State driver's license; a vehicle is required to perform the jobBecome medication certified (if required by Funder) and be trained in all required training within 90 days of being hiredMust be able to read, write, and speak EnglishMaintain current certifications with all required trainingsMust become van certifiedAbility to assist in wheelchair transfersPhysical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
